Service Description

We will go over your medical history, what has and hasn't worked in the past, and create a plan with a long-term goal and at least one short-term goal. Note: the online booking it to choose your preferred day and time. You will be contacted to confirm payment and/or insurance eligibility and then your appointment will be confirmed. You will not be charged by booking.

Cancellation Policy

The online booking is to block your preferred time only. We will be in contact to collect payment and/or confirm insurance coverage before confirming the appointment.
